CONTRACTOR AGREEMENT
Between
CODEVCO LLC
AND THE CONTRACTOR (S)
SMOKING ONLY AT DESIGNATED AREAS
The contractor is aware that there is NO INSURANCE COVERAGE, such as workman’s compensation, or general liability insurance provided. It is the contractors responsibility to provide insurance for him / her and his/ her helpers – at his / her discretion.
The contractor is responsible for his / her income tax reporting. CODEVCO LLC will issue a federal form 1099 as may be required by law.
Generally, the
contractor will be paid every day
In the event
that the contractor chooses to borrow tools or implements from CODEVCO LLC, it
is the sole responsibility of the contractor to ascertain the save working
condition of the tool or implement. Under no circumstances shall the contractor
agree to perform any task – with or without CODEVCO’s tools – that in his or her
opinion could endanger persons or property.
The contractor will be fully responsible for damage or injury caused by his or her actions.
The contractor
is aware that some of the chemicals used on the property may be considered a
health hazard by some states such as California. Material Safety Data Sheets (MSDS)
ARE AVAILABLE IN THE OFFICE.
